Summary:
Form auto alias generation not as expected
Detailed Description:
If you create a new form and leave the alias field blank, it creates the form
with no alias. If you create a second one, it'll have the alias of '1', and then
sequentially from there.

Expected action would be to generate it from the form's title (I think that's
how it used to work, but haven't tested a previous version)
