[#11733] from name, from address, subject in the "send" field are not ok in sent emails

Summary:
from name, from address, subject in the "send" field are not ok in sent emails
Detailed Description:
The email received by the site owner (and the copy for the user that filled in
the form) uses the name of the user as "from name", the email address for "from
address" and the subject field from the user.
It should use what is set in the "email results to set address(es)" fields.

My host informed me that it could lead to emails not being delivered because of
the wrong sender in email headers.

I searched where to modify code to propose a fix but didn't find. Please see if
you can fix it. Thanks.
