Detailed Description:
Hi there,

I've tried with Form Builder 0.7.3 and 0.8.1.1 on CMSMS 1.11.11. 

First I tried  to write the results as an XML-Code into an .XLS-File. After some
tests I realised, that no file header has been submitted into the xls-file.SO I
could not open it as an xls.

I tried it also with the automatically generated file-header into a .txt-file.
Same result:  the header-code is shown in the template but no header in the file
itselves.

Rows and footer works.
