[#10429] "Mail from" is unset when using smtp to send a copy of the form back to person submitting form

Summary:
"Mail from" is unset when using smtp to send a copy of the form back to person submitting form
Detailed Description:
When a remote user submits a form and elects to have a copy, "Mail from" is
blank when using smtp in CMSMailer to send a copy of the form back to person
submitting form. Consequently, the email will not get sent.  This happens if
"Which email headers should this input populate?" is set to "Reply-to".

Example Exim log:

2015-02-23 19:55:51 1YPz6d-00014D-7g <= <> H=localhost (192.168.1.70) [::1]
P=esmtp S=1975 id=ad9360ff11400c79a43d02567adc85b2@192.168.1.70

The <> should contain the Mail From field.

With a vanilla install of FB 0.8.1.1 on CMSMS 1.11.11 and 1.11.12, I'm not being
able to reproduce this. Tested both on localhost and online server. I'll need
more data to, at least try to reproduce this, if it is indeed a bug.

Happens when  "Which email headers should this input populate?" is set to
"Reply-to". We need an option to specify Mail From in this instance.

Removed the DispositionFromEmailAddressField class and added the send a copy
functionality to
FromEmailAddressField, it will send of copy of the FIRST email
Disposition it finds.
