Add a flag to the stylesheet assoc table to indicate wether this stylesheet is
attached for management purposes, but not necessarily included in the
{cms_stylesheet} call.

This is useful for conditionally loading stylesheets <!-- IFIE --> etc.  but
still having the stylesheet attached to the design for management purposes.
