Summary:
Set "From Name" to user-submitted name ("From Address" unchanged)
Detailed Description:
When a form is submitted please add the option to change the "From Name" to
match the Name of the person submitting the form.
"From Address" should be left unchanged to comply with DKIM/SPF.
This would allow users to see who a form was sent from in their mailbox email
list without interfering with DKIM/SPF checks.
