Undefined index: prefname
Detailed Description:
When installing Banners into CMSMS 1.11.6, it is a severe issue that affects
front end and back end.

However, when installing an earlier version and upgrading (to look for which
version introduced the error), it affects only the back end in 1.11.5. Banners
was installed into test version with CGExtensions 1.31.4 and no other add-on
modules to minimize potential conflicts.

This uses a recent install of WAMP with MySQL 5.5.24 and PHP 5.4.3. The server
has been configured to meet or exceed all CMSMS minimums. This has also been
installed on a production server (and removed), causing the same errors. This is
why I report it as a bug (instead of looking for help in the forum).

Notices are minor... they can be turned off and usually don't effect
functionality.
