Duplicat Hierarchy Image Issue
Detailed Description:
When you try to attach an image to a hierarchy item that has the name of an
image that already exists in the /Products/hierarchy directory it returns
error=Array in the body of the page.

The source of the admin page cut's off at that very spot as well and the rest of
the page is not loaded.

Note: It looks like it only happens when creating a new hierarchy item and not
editing it.
