on line 45

line in question is a setcookie() directive

commenting out line 56 of utils.php (echo statement) resolves the issue

Responsive file manager has been upgraded in the new 3.1.3 - does this solve
your problem?

it does, although i'm seeing some other issues at the moment that I can't
isolate - thumbnails are not showing when images are uploaded.

if uploaded through TinyMCE they don't show the thumbnail in file view, nor do
they show if reviewing using the core file manager (Content > File Manager).

also, images uploaded using the core file manager, do not display the thumbnail
image when trying to add a photo with TinyMCE editor.

I have the latest version (3.1.3), and the problem remains. I have altered
htaccess but I still get the error message described above.
