duplicate file checker
Detailed Description:
If uploading in the same session, files with duplicate filenames should be
renamed rather than overwrite (simple solution) or warn the customer and let
them cancel or rename. (more elegant solution).

Changed in 1.0.18 to add an index number. Much easier to do than the elegant
solution. I hope it's acceptable.
