When making a selection for proof approval/modification, could we add the
following:
-a title for the 'notes' fields, or default text that is deleted when they start
typing (e.g. "describe your modifications here...")
-optional messages to show for other selections (e.g. if they choose "will
resubmit" then the message "additional proof charges will apply" or something)
