Checkout does not continue
Detailed Description:
When using the speedcheckout the checkout doesn't get past the billing/shipping
adress page and when you click "Countinue step 2" the form gets emptyed and the
following error shows up:

Warning: Parameter continue is not known by module CartMadeSimple dropped in
/home/dilling/domains/dilling.qlic.nl/public_html/lib/misc.functions.php on line
1411

The checkout doesn't work for me too (see the closed bug #8827, the problem 
still exist in version 0.4.4)

Are there any solution for the not functioning order checkout problem in Version
0.4.3 and 0.4.4. The module can't be used with this big problem....
