No default/sample template
Detailed Description:
The new version of this module (8-2-09) comes without a default template.  It is
blank.

When I put {get_template_vars} in there the only module-specific objects are
gateway and formvalues.

Using print_r on gateway locks up the page.

So if you use this module, there is no payment button in the Orders confirmation
page now.

I uninstalled and re-installed the module
and had no difficulty.

I found the template in the module files.  It just doesn't get loaded when you
upgrade the module.
