Remove items flagged for removal + Promotion
Detailed Description:
When deleting products contained in the cart (Viewcart), and if they are subject
to an instant promotion. The following message appears.

"todo: handle offer with no cart index" (action.viewcart.php : line 156)


No problem if there is no promotion on the product.
