Duplicate title produces incorrect auto custom URL
Detailed Description:
Add an item title "Test"
Custom URL = Test: OK
Add another item title "Test"
Custom URL = Test-2: OK
Add yet another item title "Test"
Custom URL = Test-2-3: Not OK
Add yet another item title "Test"
Custom URL = Test-2-3-4: Not OK
etc....

Custom URLs should be Test, Test-2, Test-3, Test-4 etc.
