phpCaptcha images are cached by browser
Detailed Description:
When using phpCaptcha (Captcha module v0.4.6) with FormBuilder, if the submitted
form fails validation the captcha image does not refresh. The previous cached
image (i.e. an incorrect image) is displayed, making it impossible for the user
to correctly pass the captcha test.

This caching problem does not occur with GD Captcha or hn_captcha, but these are
uglier and less user-friendly captchas IMO.

I haven't been able to reproduce this issue. Either it has been solved already
or was specific to a particular browser/server configuration.
