Outputting static text in form submission template (or grouping radio box groups)

Sometimes one may have a question with "sub-questions" that each is responded to
with for example Yes/No/Maybe. Each "sub-question" is a label that can be output
when the form is submitted. But what about the group header?

Either letting static text be processed to the form submission template or some
other way of grouping multiple radio box groups would be excellent.
