User-defined option in radio box group
Detailed Description:
Often a group of choices with radio group or check box group may only include
the most possible options/answers to a question. Then it's good to have a last
option, like "Other:" where the user can specify if that radio box/check box is
selected.

Hi there,

I have this problem too to create a form...

But not only with the "last option"

It could be great to have an option adding a single text line at the end of each
Check/radio boxes

So CSS styling would be better (all of this inside a <div>)

Actually I'm inserting some <div>s before and after... but this is not the best
practice
